Our Office Responsibilities

• We bill your primary and if applicable your secondary insurance company. For example, Medicare and MediCal, HMO, PPO. 
• We accept cash, checks and major credit cards.
• We will send you a statement to notify you of remaining unpaid balances. No statement will be sent to you until your insurance
  carrier has met their financial responsibility.
• Our billing company will send out a statement for any unpaid balance. 

Patient Responsibilities

• Bring your driver's license (ID), health insurance card or cards & authorizations to your first appointment.     
• Inform our office of any changes with your insurance or health information.
• Be prepared for your co-pay should it apply.

If you have any questions when filling out this form, please do the best you can, our staff will assist you with your questions on the day of your visit.
